The Bulgarian National Bank issued the 5 leva banknote on July 5, 1999, as part of the fourth lev series introduced following the currency redenomination at a rate of 1000:1, aligning 1 new lev to 1 Deutsche Mark.

The obverse features a portrait of artist Ivan Milev (1897–1927) on the left, accompanied by a holographic stripe along the left edge. The reverse displays fragments from Milev's paintings, including "A Woman Harvesting," "The Bulgarian Madonna," and "Wedding of the Dragon."
